Please download and complete the application form.
Please provide copies of the following documents to the NT South Animal Management Centre by fax, mail or in person on or before 23 March 2013.
1. The completed application form.
2. Valid Dog Licence.
3. Valid Vaccination Certificate of major infectious diseases (i.e. Canine distemper virus, Canine parvovirus and Canine contagious hepatitis virus).
Candidates will be processed by the order of receiving their applications.
