This species is distinguished from similar local puffers by the oval white spots on the back. It is most abundant around shallow reefs but occurs to at least 15 m depth. Milkspotted puffers can attain 33 cm in length.

   

This fish was found buried in sand at Shelter Island, with only the eyes and upper body uncovered. It is the most abundant puffer in shallow waters.
